I actually posted about this story in a forum and were surprised that some of the readers hadn't heard of similar stories, but this happens a lot. Way more than it needs to. People should turn off their shredder, unplug it and put it away from toddlers and pets. And they should also get a shredder that senses when a hand or paw gets near it and shuts off.
